Complete your membership form in two easy steps:
Step 1: Verify your email address
- Enter your personal email address, not a district or work email address. (You will receive a verification email at this address to continue filling out the form.)
- Open the email and click the “Review Document” link to complete the Member Enrollment and Salary Deduction Authorization form. (Please note: the verification link expires after two hours.)
Step 2: Fill out the form
- Enter your information on the form. If you don’t know your CSEA Chapter Number, you can look that up here.
- Sign the form electronically using one of the three options available.
- When all the required fields are complete, tap the blue “Continue” button at the top of the form.
- Then, tap the “Agree” button to submit your form.
